Following is a list of the paperwork you will need for each of your thesis and dissertation meetings. As the student, you should obtain the paperwork for your examination in advance of your meeting, fill in appropriate parts, and bring it to your meeting. The Graduate School will also prompt some paperwork that is not listed here. All forms listed here are also available on the Graduate School’s Forms for Students page.

For any form that is to be turned in to the departmental administrative assistant, it is not necessary to obtain signatures from the chair or director of graduate studies first; the administrative assistant will obtain signatures and send them to the Graduate School.

For any document you defend within the department (proposal, thesis, dissertation, etc.), you must place a spiral-bound copy on the counter in the main office at least two weeks prior to the date of defense. Please include the date, time, room, and names of committee members on the cover page.

Please return all paperwork to the administrative secretary after your defense, including the counter copy of your proposal, thesis, or dissertation with final committee vote and data.

Master’s Degree

Application for Degree: Must be completed online. The link for the application is on myBama. You will be notified by email when it has been approved. You should apply for the degree no later than the second week of the semester in which you plan to graduate, but you may apply before that time. When applying for the Master’s, contact the Graduate Registrar, Eric Harris eharris@ua.edu or 348-8287, if MA is not listed on the application.

Completed thesis: Due mid-semester (deadlines vary by semester – Graduate School deadlines are posted in the department). Also, an electronic copy of the raw data must be submitted to the psychology department.

Thesis Proposal Meeting

Thesis Examination

  • Reminder to Committee Chair (1 copy): Give this to your thesis chair at the thesis examination meeting.
  • Ballot Form (1 for each committee member)
  • Committee Acceptance Form for Electronic Thesis or Dissertation (3 copies): Submit the signed form to the Graduate School as soon as possible after it has been signed, but no later than the date when the thesis is submitted electronically. This form takes the place of the signature page that used to appear at the beginning of the paper thesis document. One copy will be retained in your departmental file. Signatures must be original (no faxed/photocopied signatures). This is a fillable pdf form; please type instead of writing.
  • Document Rubric One copy for each committee member to be distributed when the document is sent to the committee and collected following the oral exam by the committee chair.
  • Oral Exam Rubric  One copy for each committee member to be distributed to each committee member to be distributed at the oral exam and collected following the oral exam by the committee chair.

Doctoral Degree

  • Application for Candidacy for the Doctoral Degree Form (3 copies): Submit the signed copies to the departmental administrative assistant. This is a fillable pdf form; please type instead of writing.
  • Ballot Form (1 copy for each committee member): Give these to your dissertation chair at the meeting.
  • Document Rubric One copy for each committee member to be distributed when the document is sent to the committee and collected following the oral exam by the committee chair.

Final Oral Examination

  • Ballot Form (1 copy for each committee member): Give these to your dissertation chair at the meeting.
  • Reminder to Committee Chair (1 copy): Give this to your dissertation chair at the meeting.
  • Committee Acceptance Form for Electronic Thesis or Dissertation (4 copies): Submit one original signed form to the Graduate School as soon as possible after it has been signed, but no later than the date when the thesis is submitted electronically. Copies of the Committee Acceptance Form also should be included after the title page in any bindery copies you order for your dissertation chair(s). One copy will be retained in your departmental file. Signatures must be original (no faxed/photocopied signatures). This is a fillable pdf form; please type instead of writing. An amended plan of study should be attached to this document; please notify Mary Beth Hubbard that you will need this form.
  • Bound Copy/Graduation Form (2 copies): Submit 1 completed copy to the departmental administrative assistant. Retain the second copy for your records.

  • Final Oral Exam Rubric (1 copy for each committee member): Forms should be collected by committee chair and given to Director of Graduate Studies.
  • Oral Exam Rubric One copy for each committee member to be distributed to each committee member to be distributed at the oral exam and collected following the oral exam by the committee chair.
  • Document Rubric One copy for each committee member to be distributed when the document is sent to the committee and collected following the oral exam by the committee chair.
