Thesis and Dissertation Paperwork

As the student, you should obtain the paperwork for your examination in advance of your meeting, fill in appropriate parts, and bring it to your meeting.  Below is a list of the paperwork you will need for each of your thesis and dissertation meetings.  For each item of paperwork, it is stated below what action should be done. The forms are also linked below. For any form that is to be turned in to the Departmental Administrative Assistant, it is not necessary to obtain the Chair's or the Director of Graduate Studies signature first. Signatures will be obtained by the Departmental Adminstrative Assistant.

Thesis Proposal Meeting (fall or spring semesters; may be conducted in summer only if all committee members agree to meet).
Thesis Examination (fall and spring semester only)
  • Reminder to Thesis/Dissertation Chair (1 copy).  Give this to your thesis chair at the thesis examination meeting.
  • Signature page of your thesis (multiple copies, prepared by the student according to graduate school guidelines).  Save the signed copies to insert into your thesis when you submit it to the graduate school.
Mini-proposal Meeting (fall or spring semesters; may be conducted in summer only if all committee members agree to meet).
Preliminary Examination (fall and spring semester only)
Final Oral Examination (fall and spring semester only)
  • Ballot Form (5 copies).  Give these to your dissertation chair at the meeting.
  • Reminder to Thesis/Dissertation Chair (1 copy).  Give this to your dissertation chair at the meeting.
  • Signature page of your dissertation (2-4 copies, prepared by the student according to graduate school guidelines).  Save the signed copies to insert into your dissertation when you submit it to the graduate school. Also, copies of these should be included in your 2-3 bindery copies for the department.
  • Bound Copy/Graduation Form (2 copies). Submit 1 completed copy to the Departmental Administrative Assistant.  Retain the second copy for your records.
